Vietnam war veteran and orphan, Lincoln Clay, returns to New Orleans only to be attacked by the Italian Mob who tries to kill him. After joining with a few allies, he creates the Black Mob and seeks to take revenge on those who tried to kill him.

The game has a lot of new features like an interrogation system and a more refined hand to hand combat system. It also features a cover mode, but it encourages more gunplay than stealth. The players can take over locations run by the Italian Mob and control it.
